Job Description:
We are seeking an experienced Case Manager to join our team in Los Angeles. The Case Manager will be responsible for providing comprehensive case management services to clients in need of assistance. The Case Manager will assess the needs of clients, develop individualized service plans, and coordinate services to ensure that clients receive the necessary support. The Case Manager will also provide advocacy and support to clients and their families.
Responsibilities:
• Assess the needs of clients and develop individualized service plans
• Coordinate services with other agencies and organizations
• Provide advocacy and support to clients and their families
• Monitor client progress and adjust service plans as needed
• Maintain accurate records and documentation
• Participate in team meetings and other professional development activities
• Provide crisis intervention and support
• Develop and maintain relationships with community partners
• Participate in outreach activities to identify potential clients
Qualifications:
• Bachelor’s degree in social work, psychology, or related field
• At least two years of experience in case management
• Knowledge of community resources and services
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and problem-solving skills
• Ability to work with diverse populations
• Valid driver’s license and reliable transportation
